*Only active clients of calmYOUnity are permitted to submit a nomination at this time.

*Nominee must be willing to sign a calmYOUnity Service Agreement and Damage Liability Waiver in order to receive services.

​

*If nominee will be receiving Rescue ME! services, nominee must understand that they are responsible for the cost of their groceries.
